چکیده

The effects of open heart surgery on various parameters of thyroid-pituitary function were studied in 11 patients. The pattern of hormonal changes was variable. T4 concentration increased during anaesthesia and bypass, following which it returned to normal once haemodynamic stability had been regained. The pattern of change in T3 concentration did not follow consistently that of T4 and the proportional increase in concentration was not as great. There was a significant reduction in T3 concentration with the onset of haemodynamic stability. For individual patients there was no obvious correlation between changes in the concentrations of TSH, T3 and T4. The increases may be explained by release of T3 or T4 from extrathyroid tissue.

Non-thyroidal illness syndrome and cardiopulmonary bypass in children with congenital heart disease

Abstract Background: The thyroid hormones influence on all metabolic pathways. After heart surgery using cardiopulmonary bypass (CPB), serum T3 decreases and remains low for at least 24 hours. Several studies on pediatric have reported reduction of thyroid hormones after heart surgery. This study aimed to investigate the status of thyroid function tests in children with CPB surgery. Metho...

Pituitary Apoplexy Following Mitral Valvuloplasty

Pituitary apoplexy is a rare but potentially life-threatening clinical syndrome caused by the sudden enlargement of a pituitary adenoma secondary to hemorrhage or infarction. Pituitary apoplexy after cardiac surgery is a very rare perioperative complication. Factors associated with open heart surgery that may lead to pituitary apoplexy include hemodynamic instability during cardiopulmonary bypa...

Problems of Cold Agglutinins in Cardiac Surgery: How to Manage Cardiopulmonary Bypass and Myocardial Protection

Cold agglutinins are of unique relevance in cardiac surgerybecause of the use of hypothermic cardiopulmonary bypass (CPB). Cold autoimmune diseases are defined by the presence of abnormal circulating proteins (usually IgM or IgA antibodies) that agglutinate in response to a decrease in body temperature. These disorders include cryoglobulinemia and cold hemagglutinin disease.Immunoglobulin M aut...

Clinical review: Thyroid hormone replacement in children after cardiac surgery – is it worth a try?

Cardiac surgery using cardiopulmonary bypass produces a generalized systemic inflammatory response, resulting in increased postoperative morbidity and mortality. Under these circumstances, a typical pattern of thyroid abnormalities is seen in the absence of primary disease, defined as sick euthyroid syndrome (SES). The presence of postoperative SES mainly in small children and neonates exposed ...

The Pituitary-Thyroid Axis in Acromegaly

The pituitary-Thyroid axis of 12 Acromegalic patients was evaluated by measurement of the concentration (Total and free of thyroxine T4, T3 and revers T3 (rT3) and TSH, GH and prolactin before and after in stimulation with TRH. Using an ultrasensitive method of TSH measurement (IRMA) basal serum TSH levels of patients (0.76, 0.07-l.90mlU/L) were found slightly but significantly (P < 0.01), low...
